This compromised case was treated by Anna Martyrosian, doctor from my clinical team.
The case is not so easy one, class III, cross bite, open bite tendency, huge difference between upper and lower transverse dimension. No TMJ problems.
I think, it’s not possible to fully correct cross bite without SARPE in traditional way (while MARPE in adults is still experimental option). If we try to correct by dentoalveolar movement, the molar roots will be out of bone or crowns should be tipped buccally with «hanging» palatal cusps and bite opening due to premature contacts. If we expand with SARPE, too much space will be created, that will lead to longer treatment to mesialize upper molars. The second option is to create space for Implants after expansion and distalization, that is also not perfect.
So, together with the patient we decided to have compromise, not correcting cross bite completely (leave in molar region).
I don’s say it’s ideal plan, but I would do it for my sister, for example.
